Your health information is covered by a separate notice
This page is about the website. The medical information the practice holds about you as a patient is governed by our Notice of Privacy Practices, which sets out how we may use and disclose protected health information and what rights you have over it. The two documents are separate on purpose, and neither one replaces the other.
Information you give us
If you submit the contact form, we receive the name, email address, telephone number, and message you enter. That information is delivered to the practice by email through our forms provider, Formspree. It is used to respond to your enquiry. We do not sell it, rent it, or share it for anyone else’s marketing.
Please do not submit medical records, diagnosis information, or other detailed health history through this form. It is a general enquiry form, not a secure patient portal and not a clinical record, and it is not the right channel for protected health information. To discuss anything clinical, or to request medical records, call the office at 708.371.3090.
Information collected automatically
Very little. This website loads no Google Tag Manager, no Google Analytics, and no advertising tag. It sets no tracking or analytics cookie, which is why you are not asked to accept one. What remains is the ordinary server log our host keeps for every website it serves: the address of the page requested, the time, and technical details such as browser type and IP address.
What we do not send to advertisers
We do not transmit personal or health information to any advertising or analytics provider. No name, email address, telephone number, or message you enter on this site is passed to Google or to any comparable service, in any form, hashed or otherwise. This practice runs no advertising measurement on this website at all.
Using this site is not a HIPAA authorization
Visiting these pages, or sending us a message through the contact form, gives no permission to use or disclose your protected health information. The U.S. Department of Health and Human Services, Office for Civil Rights has stated expressly that a website cookie banner does not satisfy the requirements for a valid HIPAA authorization, and neither does any other click on a website. A HIPAA authorization is a separate document that you sign, and it is described in our Notice of Privacy Practices.
How long it is kept
Enquiries you send us are retained by the practice for as long as needed to respond to you and to meet our record-keeping obligations. Your medical record is retained under the schedule described in the Notice of Privacy Practices, which is a separate matter from anything this website holds.

- Vercel — website hosting, including standard server logs. See Vercel’s privacy policy.
Your rights
Depending on applicable law, you may have rights concerning your personal information, including rights to request access, correction, or deletion. These rights may be limited where we are required to retain information under HIPAA, medical record requirements, litigation obligations, or other applicable law. We will review and respond to verified requests as required by applicable law.
